Sometimes home is the best place to spend the last days of your medical health journey surrounded by loved ones. We offer a range of services to support you spend time with your loved ones in collaboration with the community palliative care team, including emotional support, and wellbeing management. At Eternity Care and Support Services we use care plans which include your wishes and used to tailor the supports needed to achieve goals of care and meet palliative care needs. Palliative Care Support services provided can be during the day, overnight care and 24-hour care. Palliative Supports provided include:

• Assistance in pain management.
• Medication management.
• Multidisciplinary care with your palliative care team and other treating professionals to provide appropriate care.
• Participants and families going through Palliative Care often choose to be surrounded by loved ones, in the comfort of their own home, with care services provided by a team of Nurses and Support Workers.
• We offer a range of services to support you spend time with your loved ones in collaboration with community palliative care team, including emotional support, and wellbeing management. At Eternity we can create a care plan for provision of care which includes your wishes> care is tailored to goals of care and needs (during the day, overnight care and 24-hour care).
