High-Intensity Home Care

High-intensity personal activity support is an NDIS term that describes the supports required to support NDIS participants with complex health issues. This is also commonly known as Nursing and Clinical Services or High Intensity Supports that require either specialised, highly skilled, and experienced, support staff or Registered Nurses to administer the supports required by participants. At Eternity, our skilled support workers and registered nurses can support you and collaborate with you to provide centred person care. Our in-home support teams are trained and assessed regular on their the skills/ knowledge to deliver complex, high intensity support.

High Intensity Activities we assist

Management of Tracheostomies

our RNs have experience in creating care plans and providing care for participants with temporary, or new, or established tracheostomy. Cares provided include mouth care, suctioning, cleaning of equipment, oxygen delivery, skin/stoma management and emergency equipment management, In addition, our experienced support workers can manage and provide safe tracheostomy tube care under supervision of our intensive care trained nurses. The registered nurses can manage all aspects of airway management and tracheostomy care. What sets us apart from other companies is that our registered nurses undergo annual advanced life support training and are still bedside practicing RNs who have evidence-based knowledge in respiratory conditions management. For your peace of mind, we have on call RNs who can present to your home to trouble shoot and assess participants if there are concerns raised by support workers.

Complex Bowel care

our registered nurses provide specialised care to promote healthy gut, bowel nursing care plans, medications, and dietary management when providing the Eternity complex bowel care service following best practice guidelines and medical instructions from your treating health professionals.

Bladder management and urinary incontinence

our RNs can create care plans and assist you to manage devices (SPC, IDC, uridomes, and self-catheterisation devices). Our RNs together with our support workers can assist you to manage urinary incontinence and prevent pressure injuries as well as incontinence associated dermatitis by performing regular skin assessments tailored to your condition.

Feeding and Naso-gastric tubes

our intensive care trained registered nurses are able to assist with feeding care plans, regular feeding, parenteral diets (Intravenous glucose, TPN, vitamins and medications), observations such as glucose levels, dietary modifications for oral foods, modified diets or thickening of fluids, dysphagia management, enteral feeding (gravity enteral feeding, intermittent enteral feeding, bolus enteral feeding and 24 hour enteral feeding), feeding device access and equipment management (NGT -nasogastric tube, PEG tube, JJ Tube, Orogastric tube and reinsertion of NGT tubes under medical orders). Eternity can assist you to manage your nutritional needs and work with speech pathologist/ dieticians to prevent complications.

Any other high-intensity assistance

Our intensive care nurse specialists can assist with ventilator management, pre-operative management with your GP or hospital, heart failure medication management, alcohol withdrawal or drug withdrawal under medication/ treatment plan, sleep apnoea management, complex medication management and other cares that require skilled nurses.

Subcutaneous and Intramuscular injections

Our RNs can assist you in administer subcutaneous and intramuscular injections in the comfort of your own home as per your treating health professional’s instructions.

Complex wounds

our RNs can help with managing your complex wounds and make recommendations for products. Eternity RNs follow best practice wound guidelines and work with clinical wound specialists to manage your condition.
